Konsultasyon
Consultation tracking covering both intradepartmental consultations and cases referred in from outside laboratories.
Purpose
Two linked workstreams. Intradepartmental: patterns, timing, and value of within-department consultation. External: characterize referred-in cases — a distinctive workload of harder cases with incomplete information, slower turnaround, and higher variability.
Data used
The consultation log in this repo (external stream filters source = external), Kris reports for the final issued diagnosis, and outside-lab narrative text parsed through Extract Report Text. Integrates with the LIS (Patoloji Bilgi Yönetim Sistemi (LIS)). Sites: konsultasyon.patoloji.dev, konsultasyon.patoloji.app.
Methods
Case-mix description by organ, specimen type, and originating institution type; turnaround from receipt to final report; agreement versus disagreement with the referring diagnosis; identification of cases where outside material is insufficient and must be re-requested. Statistics through ClinicoPathJamoviModule, agreement via meddecide.
Current state / open questions
External-consult stream: retrospective cohort identification complete, metadata cleaning in progress, agreement coding rubric drafted, analysis at planning stage. The intradepartmental chapter is currently a stub — links only, no recorded status. [unverified] Documented pitfalls: “disagreement” bundles refinement, reclassification, and true discrepancy, and collapsing the three hides the meaningful signal; referring-institution identity is sensitive, so analyses aggregate at institution-type level (academic / private lab / community hospital) and never by name.
